Connected Patient Experience Across the Continuum uses a specific case study (Bellin Health of Green Bay, Wisconsin) to offer a compelling example of how a group of three community fitness centers made the transition to medical-based centers that contributed to the continuum of care within the Bellin health system. The DVD discusses the existing tools that collectively the three centers employed to help facilitate the transition, as well as explains how they were able to change their focus/priorities, culture, resources, and systems/processes paradigm from “hospital-centric” to “health networks.”
